What Invisalign can now handle — and what still needs braces

The line has moved. Moderate to severe crowding, moderate bite correction, extraction cases and adult retreatment can now often be treated with aligners, and in selected cases even some impacted canines, with auxiliaries. What makes this possible is the toolkit around the trays: attachments, patient-worn elastics, interproximal reduction (IPR), bite ramps and button-and-elastic auxiliaries. Braces still have the advantage for very severe rotations and complex root movements, major back-tooth intrusion, surgical orthodontics, some skeletal corrections and, importantly, patients who will not reliably wear aligners 20–22 hours a day, because fixed braces work whether or not you cooperate. Timelines follow complexity: simple aligner cases can finish in around six months, severe cases take 18–24 months, and retreatment after mild relapse is often among the shortest courses.

When braces have the advantage

Beyond the complex cases above, braces are often the better fit for:

  • Some growing children and teenagers, where predictability matters most
  • A tighter budget, since metal braces usually cost less

When Invisalign has the advantage

  • Mild to moderate crowding or spacing
  • Adults who want discreet treatment
  • No diet restrictions, because the aligners come out to eat
  • Easier cleaning and fewer emergency visits for broken brackets or poking wires
  • A digital plan that previews the expected tooth movement before you start

Braces, in turn, can be harder for:

  • Adults in client-facing roles who do not want visible brackets
  • People whose cheeks and lips are easily irritated by wires
  • People who find cleaning around brackets difficult, since brackets trap plaque

Treatment time

  • Simple aligner cases: about 4–8 months
  • Comprehensive aligner cases: about 12–24 months
  • Simple braces cases: about 6–10 months
  • Comprehensive braces cases: about 18–30 months

In comparable cases, braces and aligners take roughly similar time. The plan, not the appliance, sets the timeline.

Comfort

  • Invisalign: mild tightness for a day or two after each new aligner; little soft-tissue irritation
  • Braces: soreness for a few days after each adjustment; brackets can rub the cheeks and lips at first

Food restrictions

  • Invisalign: none while the aligners are out, but you must brush before putting them back in. See what to eat with Invisalign
  • Braces: avoid popcorn, hard sweets, sticky caramels, nuts, ice, whole apples and hard breads, which can break brackets. Living with braces has practical tips

Total cost over treatment

Braces can look cheaper upfront, but check whether the quote includes:

  • Adjustment visits (usually every 4–8 weeks)
  • Replacement of broken brackets
  • Emergency visits for poking wires
  • Retainers after treatment (often a separate cost)

Aligner quotes are usually packaged by tier. Ask which review appointments, refinements and retainers are included.

What "comprehensive" should include

For braces:

  • Initial records (X-rays, photographs)
  • Brackets and wires
  • All adjustment visits
  • Removal at the end of treatment
  • Retainers (check whether included)

For Invisalign:

  • A digital scan
  • The 3D treatment plan
  • All aligners for the case
  • All review appointments
  • IPR if needed
  • Attachment placement and removal
  • Refinements and retainers (check what the package includes)

Real talk on compliance

The "Invisalign doesn't work" complaint usually comes from patients who did not wear their aligners 20–22 hours a day. Compliance is critical: if you doubt you will keep the aligners in, say so at the assessment. Living with Invisalign covers the daily routine.

Insurance coverage

Adult orthodontics is rarely covered by UAE plans. Some higher-tier plans include a limited orthodontic benefit, usually with a lifetime cap and pre-authorisation, so check your policy before starting.

Choosing between them

For many adults with mild to moderate crowding or spacing, aligners and braces reach similar results, and the choice comes down to appearance, comfort, daily routine and cost. For complex cases, surgical orthodontics, or anyone unlikely to wear aligners consistently, braces are often the more predictable route. An orthodontic assessment with X-rays and a scan is the only way to know which suits your teeth. The Invisalign page and braces page describe each treatment.

Practical decision guide

Orthodontics works by controlled tooth movement over time. The best plan balances alignment, bite correction, gum safety, compliance, and lifelong retention rather than chasing the shortest timeline.

Check this first

  • Bite classification, crowding, gum thickness, jaw growth, wisdom teeth, and any history of relapse.
  • Whether aligners, braces, expanders, elastics, or a combined cosmetic plan is most predictable.
  • The retention plan after treatment, because teeth continue to move throughout life.

When to book sooner

  • A child has crossbite, severe crowding, mouth-breathing habits, thumb sucking, trauma risk, or jaw asymmetry.
  • Aligners are not tracking, attachments are lost, elastics are not being worn, or teeth feel unusually mobile.
  • You are considering direct-to-consumer aligners without in-person periodontal and x-ray assessment.

Topic-specific notes

  • For cost decisions, compare itemised treatment plans rather than headline prices. The clinically important inclusions are diagnosis, imaging, material choice, temporaries, follow-up, maintenance, and what happens if treatment needs revision.
  • For orthodontics, compliance and retention are part of the treatment, not afterthoughts. If aligners, elastics, or retainers are not worn as prescribed, the biology will not follow the digital plan.

Questions to ask at the appointment

  • What is the real treatment goal: cosmetic alignment, bite correction, or both?
  • How many refinement stages are included, and what happens if teeth do not track?
  • Will I need fixed, removable, or combined retainers after treatment?

Dubai patient note

For Dubai orthodontic treatment, ask whether the provider is DHA-licensed as an orthodontist or is a general dentist providing aligners, and confirm what is included in the quoted fee.
